RESPONSIBILITIES:

* Provide personal care assistance to residents (bathing, dressing, grooming, etc.)
* Monitor and report changes in residents' health or behavior
* Assist with meals, mobility, and toileting
* Maintain a safe, clean environment in accordance with facility standards
* Collaborate with nursing staff and other healthcare team members
* Document care provided in electronic medical records
