Registered Nurse

Primary Responsibilities: Perform comprehensive assessment of high risk members. Collaborate with primary care providers to ensure the implementation of an individualized, comprehensive care plan with specific interventions designed to engage the member. Address, identify, and continuously reassess cost-efficient, appropriate levels of care.

On call rotation 2 nights a week/ every third weekend. Expectations 4 -5 patients a day
License Requirements: WV/Compact license
Mileage Reimbursement: .54 per mile
County Covered: Harrison, Barbour, Upshur, Lewis, Randolph, Braxton, and Webster
Preferred Skills: Hospice and HCHB experience required. Candidate will be required to complete admission, routine, PRN, and death visits.
