* Active LPN license
* Graduate of accredited practical nursing program
* BLS
* Previous home health or clinical experience preferred
* Valid driver’s license and reliable transportation
* Strong communication and time management skills

RESPONSIBILITIES:

* Provide skilled nursing care in patients’ homes
* Monitor vital signs and patient condition
* Administer medications and treatments as ordered
* Perform wound care and basic procedures within scope
* Educate patients and caregivers on care plans
* Document visits and patient progress accurately
* Report changes in condition to RN or provider
* Follow infection control and safety guidelines
