Position Overview
The Licensed Practical Nurse (LPN) provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse. This role is ideal for an LPN who enjoys hands‑on patient care, autonomy, and building meaningful relationships with patients and families.
Key Responsibilities

Provide skilled nursing services in accordance with the Plan of Care
Perform treatments and nursing procedures per physician orders and agency policy
Observe, assess, and document patient responses to care
Educate patients and caregivers on disease processes, medications, and home care techniques
Communicate changes in patient condition to the supervising RN and physician
Maintain accurate, timely clinical documentation in compliance with Medicare/Medicaid standards
Adhere to infection control, HIPAA, and safety policies
Participate in in‑services and ongoing education

Qualifications

Active Missouri LPN license
Minimum of 2 years nursing experience (home health preferred)
Strong clinical and interpersonal skills
Ability to work independently in the field
Reliable transportation
Dependable, organized, and patient‑focused
