Job Description
We are currently seeking an experienced Licensed Practical Nurse (LPN) for a contract opportunity within a skilled nursing/long-term care setting.

The LPN will provide direct nursing care to residents, work closely with RNs, CNAs, physicians, and other healthcare professionals, and ensure that residents receive safe, compassionate, and quality care throughout the assignment.

Responsibilities
Provide direct nursing care to residents according to established care plans and facility policies.
Administer medications and treatments as ordered by physicians and according to applicable regulations.
Monitor residents' conditions and report changes to the appropriate healthcare provider.
Perform nursing assessments and document resident status, interventions, and outcomes accurately.
Assist with wound care, treatments, and other nursing procedures within the LPN scope of practice.
Communicate effectively with residents, families, physicians, RNs, CNAs, and other members of the healthcare team.
Maintain accurate and timely clinical documentation.
Follow infection-control, safety, and facility policies and procedures.
Respond appropriately to changes in resident condition and escalate concerns when necessary.
Assist with admissions, transfers, discharges, and other required nursing documentation.
Provide compassionate support and maintain resident dignity, privacy, and confidentiality.
Collaborate with the interdisciplinary team to support quality resident care.

Candidate Requirements
Active and unrestricted LPN license applicable to Alabama.
Previous experience in Skilled Nursing Facility (SNF), Long-Term Care (LTC), Rehabilitation, or similar setting preferred.
Strong clinical assessment and documentation skills.
Ability to work independently and as part of a healthcare team.
Reliable attendance and ability to work the assigned schedule.
Must be available for the full contract period.
