JOB SUMMARY
The LPN plays an integral role on the leadership team at Peaceful Lodge. He/she will provide direction and leadership to our team of professional caregivers. This includes overall responsibility and accountability for the strategies of quality nursing services and overall medical operations of the Peaceful Lodge community. The LPN will also be responsible for training, coordination of resident care, conducting pre-assessments and assessments, management of medication, and being the liaison between the residents/families and physicians. The LPN coordinates and supervises nursing personnel to deliver nursing care which will ensure that resident’s needs are met in accordance with the professional standards of practice through physician orders, community policies and procedures, and federal, state and local regulations and established nursing best practices.
JOB FUNCTION
Provide personal assistance, medical attention, emotional support, or other personal care to residents as specified in their care plan.
Enter, transcribe, record, store, or maintaining information in our Rtask Case-management system.
Set and administer prescribed medications.
Observe residents, charting and reporting changes in patients' conditions, such as adverse reactions to medication or treatment, and taking any necessary action.
Train and supervise the work of unlicensed personnel and provide the necessary required training as delegated by the Registered Nurse.
Serves as the main go to person daily for resident issues, observations and follow up on medical cares and conditions.
Work as part of a healthcare team to assess patient needs, plan and modify care, and implement interventions.
Evaluate nursing intervention outcomes, conferring with other healthcare team members as necessary.
Ability to make decisions and solve problems independently.
Make appointments, keep records, or perform other clerical duties.
Conducts self in a professional and ethical manner in all work-related functions.
Demonstrates productive individual and teamwork behaviors.
Prevents crisis at the lowest level of escalation.
Monitors resident’s general physical and mental health needs from a nursing perspective and ensure cares plans reflect changing conditions.
Adheres to all company policies, procedures and processes which include but is not limited to Data Privacy Practices/HIPPA
Writes progress notes, summaries, and discharges in our Rtask system.
Organize, plan, and prioritize work in the most effective and efficient manner.
Supervise care staff to maintain the cleanliness and comfort of each patient, including bathing, feeding, and dressing.
Conduct Assessments and Pre-assessments of all potential clients.
Assist RN in 14- and 90-day reviews.
Meets with case workers as needed and act as a back up to the RN Supervisor.
REQUIREMENT
A bachelor’s degree in nursing or related field is Preferred.
5+ years of nursing experience with at least 2+ years in a supervisory role
Experience supervising a team of medical professionals – PCAs, CNAs, LPNs, and/or Nurse Practitioners
General knowledge of the state of Minnesota assisted living compliance – including licensing, auditing, and reporting.
Must have the ability to deal tactfully with personnel, residents, families, medical doctors, vendors, and community members.
Ability to work in a diverse working environment including diverse residents.
Experience working with older adults in a senior living and/or assisted living capacity is preferred.
Ability to work in a team environment.
Strong communication and interpersonal skills
Ability to work in a fast-paced working environment and prioritize.
Advanced knowledge in the Microsoft Suite – Words, Excel, and PowerPoint
