Physical Therapist – Home Health (Contract to Hire)
Location: Greenville, AL
Schedule: Mon–Fri | 8:00 AM–5:00 PM
Hours: 40 hrs/week
Mileage: $0.49 per mile

Job Overview:
Seeking a licensed Physical Therapist to provide home health services in the Greenville, AL area. The PT will evaluate patients, develop treatment plans, and deliver rehab services to improve mobility, function, and independence in the home setting.

Coverage Area:
Butler, Lowndes, Crenshaw, Covington, and Wilcox counties

Responsibilities:
Perform PT evaluations and treatments per physician orders
Develop and adjust individualized treatment plans
Provide therapeutic exercises and functional rehab
Document care accurately and on time
Communicate with physicians, care teams, patients, and families
Supervise PTAs and aides as required

Active Alabama PT license (compact accepted)
Home health experience preferred
OASIS, wound care, and HCHB experience a plus
