Join Our Telehealth Team: Family Nurse Practitioner Wanted!

PathCare Health is seeking a Family Nurse Practitioner to join our growing concierge telehealth team. We provide virtual primary care, post-discharge care, chronic care management, home care evaluations, and medical documentation for patients throughout New York.

This is a full-time Monday-Friday position with no overnight shifts. You'll work with a dedicated medical assistant and administrative team so you can focus on patient care.

NPI must BE Medicaid enrolled

Responsibilities

Conduct telehealth visits (video and phone)
Evaluate patients and develop treatment plans
Complete documentation and required medical forms
Coordinate with home care agencies, specialists, and hospitals
Prescribe medications when appropriate
Work collaboratively with care managers and support staff
