The LVN Care Coordinator supports value-based care initiatives by conducting telephonic outreach to members, closing quality care gaps, and promoting preventive care. This role works closely with providers to improve quality metrics, enhance patient outcomes, and manage high-risk patient populations.

Key Responsibilities:

Identify and address quality gaps across assigned patient panels
Support primary care providers with scheduling and care coordination
Perform telephonic outreach for preventive care, medication adherence, and chronic care management
Utilize reports and tools to track care gaps and quality performance
Provide patient education and counseling using standard protocols
Communicate effectively with healthcare teams to support coordinated care
Act as a subject matter resource for care transitions and quality measures
Requirements:

Minimum 3 years of clinical or home health experience
BLS Certification
Active LVN/LPN license
High School Diploma or equivalent
