A primary care physician opportunity is available in Stockton, CA with a PACE (Program of All-Inclusive Care for the Elderly) organization serving frail seniors age 55 and older who meet nursing home level of care but continue living at home.
The role. The physician provides comprehensive primary medical care to enrolled PACE participants, including history and physicals, lab and diagnostic ordering and interpretation, functional and mental status evaluations, and management of acute and chronic illness, medication adjustment, and specialist referral. The role functions as a member of the interdisciplinary team (IDT), participating in initial, semi-annual, unscheduled, and annual assessments as well as daily morning updates. The physician also serves as supervising physician for on-site nurse practitioners and/or physician assistants, provides inpatient attending coverage when participants are hospitalized or admitted to a nursing facility, and participates in QAPI activities and organizational committees. The position reports to the Medical Director, coordinating with the Program Director for Clinic.
What we're looking for.
• MD or DO; board certified or board eligible in Internal Medicine or Family Practice
• Active California medical license and current DEA registration
• Minimum one (1) year documented experience working with a frail or elderly population
• Current CPR/BLS certification
• Must obtain staff privileges at contracted hospitals prior to start
• Must be medically cleared for communicable diseases with immunizations current prior to direct participant contact
• Advanced certification or added qualification in Geriatric Medicine (ABIM/ABFP) preferred
