Join Our Pediatric Care Team in Peabody!

Perform clinical and administrative duties as part of the team in a Patient Centered Medical Home model pediatric primary care practice. The Primary Care Office Nurse is responsible for assisting in the delivery of health care and patient care management.

Duties and Responsibilities:

Patient Care

Room patients including vitals and review of patient medications and allergies

Review and document developmental, behavioral, and health risk screening

Perform and document vision and hearing screening

Perform in-office laboratory tests and collect and handle patient samples

Administer and document vaccinations and injections

Assist with examination or procedures when necessary

Triage patient phone calls and sick visits with established guidelines

Call patients with test results and instructions

Patient education/teaching



Care Coordination

Monitor requests for prescription refills and document refill encounters

Complete prior authorizations

Complete health forms

Track all referrals and discharges with follow up and documentation

Review medical records and document history

Perform patient outreach



Administrative

Order supplies, medications, and vaccines

Complete all responsibilities of Vaccine Coordinator including usage reporting, ordering, monitoring temperature logs and uploading data

certified LPN or RN
strong telephone skills
multi-lingual a plus
