Job Description

Teamware Solutions is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) for a contract opportunity in St. Albans, Vermont. The selected candidate will provide quality nursing care, monitor patients’ health conditions, administer medications as authorized, and collaborate with the healthcare team to ensure patient safety and well-being.

Responsibilities

Provide direct patient care according to established care plans and nursing protocols.

Monitor and document vital signs and changes in patients’ health conditions.

Administer medications and treatments in accordance with physician orders and facility policies.

Assist patients with activities of daily living (ADLs) as needed.

Observe, document, and report changes in patient conditions to the appropriate healthcare professionals.

Maintain accurate and timely nursing documentation and patient records.

Follow infection control procedures and maintain a safe care environment.

Collaborate with RNs, physicians, and other healthcare professionals.

Protect patient privacy, dignity, and confidentiality.

Provide compassionate support to patients and their families.

Requirements

Active and valid LPN license eligible for practice in Vermont.

Previous nursing experience preferred.

Current BLS certification, as required by the facility.

Knowledge of medication administration, patient monitoring, and nursing care procedures.

Strong communication, documentation, and interpersonal skills.

Ability to work the assigned evening shift.

Ability to meet facility credentialing, background check, and onboarding requirements.
