Job Summary
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our interdisciplinary rehabilitation team in a skilled nursing and post-acute care setting. The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide therapeutic interventions designed to restore function, improve mobility, relieve pain, and promote overall quality of life.
The ideal candidate is patient-focused, team-oriented, and committed to delivering high-quality rehabilitative care to geriatric and short-term rehabilitation patients.
Responsibilities
• Evaluate residents and patients to determine functional limitations and rehabilitation needs
• Develop and implement individualized treatment plans based on physician orders and clinical assessments
• Provide therapeutic exercises, gait training, balance training, mobility assistance, and pain management interventions
• Monitor patient progress and modify treatment plans as appropriate to achieve optimal outcomes
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to coordinate care
• Educate patients, families, and caregivers on therapy goals, exercises, safety precautions, and discharge planning
• Maintain accurate, timely, and compliant documentation in accordance with facility, state, and federal guidelines
• Participate in care planning meetings and interdisciplinary team discussions
• Ensure a safe and supportive environment for residents and patients during treatment sessions
• Maintain compliance with professional standards, policies, and regulatory requirements
Requirements
• Active Physical Therapist (PT) license in the applicable state
• Graduate of an accredited Physical Therapy program
• Current CPR certification preferred
• Previous experience in skilled nursing, rehabilitation, long-term care, or post-acute settings preferred
• Strong clinical assessment and treatment planning skills
• Excellent communication and interpersonal abilities
• Ability to work effectively both independently and within a collaborative healthcare team
• Compassionate approach to patient care with a commitment to improving patient outcomes
