Overview:
The Community Health Outreach Manager oversees community‑based medical outreach services and supervises outreach staff, including Community Health Representatives and patient advocacy roles. This position focuses on care coordination, patient assessments, care planning, health promotion, and collaboration with providers and community resources to improve patient outcomes.

Key Responsibilities:
Supervise outreach and community health staff
Conduct patient assessments and develop care plans
Coordinate care with providers, hospitals, and community programs
Perform limited home visits (non‑skilled) as needed
Support population health initiatives (e.g., diabetes programs)
Utilize EHR for documentation and care coordination
Lead health education and prevention efforts

Requirements:
Active California RN license
Minimum 2 years experience in nursing with public, community, or home health experience
Valid CA driver’s license and insurable driving record
Strong leadership, communication, and care coordination skills
