Join Us: Clinical Care Coordinator for Home Health!

Silver Haven Home Health LLC is seeking a Clinical Care Coordinator to support the intake and admission process for our growing home health agency. This position will review incoming referrals, verify insurance benefits and eligibility, communicate with patients and referral sources, and help ensure patients are appropriate for home health services prior to admission. This will be an offsite position.

Receive and review incoming home health referrals.
Review referral documentation for completeness and medical necessity.
Verify patient insurance eligibility, benefits, and coverage.
Contact insurance companies as needed to confirm benefits.
Communicate with patients, families, physicians, hospitals, and referral sources.
Coordinate accepted referrals with the clinical team for timely admission.
Obtain missing documentation and follow up on incomplete referrals.
Maintain accurate referral and patient information in the EMR.
Track referrals from receipt through admission or non-admit.
Communicate referral status and barriers to agency leadership.
