Job Description
Summary:
We are seeking an experienced and self-sufficient Home Health Physical Therapist to provide in-home therapy services across the High Desert region of Southern California. This traveler-friendly position offers autonomy, stability, and meaningful impact, backed by guaranteed hours and efficient routing.

Responsibilities:
Provide skilled physical therapy services to homebound patients across designated service areas

Evaluate, plan, and implement patient-specific treatment plans and interventions

Maintain accurate documentation using EPIC and Oasis systems

Educate patients and caregivers on treatment plans, exercises, and safety

Collaborate with the interdisciplinary care team to monitor patient progress

Maintain compliance with state, federal, and facility regulations

Travel between patient homes with attention to schedule and mileage

Key Requirements:
Must have 1 year of recent Home Health PT experience

Experience with OASIS documentation required

BCLS certification required

Reliable transportation required (mileage reimbursed per IRS standards)

Must be able to visit up to 6 patients/day and meet 30 productivity points

Local candidates (within 50 miles) accepted

No block scheduling; no floating; no on-call requirements

Guaranteed hours—traveler will not be called off

Qualifications
Education:
Graduate of an accredited Physical Therapy program

Experience:
Minimum 1 year of Skilled Home Health experience required
Experience managing a full caseload across a wide geographic area preferred

License:

Valid California Physical Therapy license

Pending license accepted

Certifications:

BCLS (Basic Life Support) – required

Skills:
OASIS documentation proficiency

Time management and ability to work independently in the field

Strong interpersonal and patient education skills

Comfortable navigating varied geographical zones with occasional 50+ mile radius

Familiarity with EPIC EMR preferred

Must have 1 year of recent Home Health PT experience
Valid California Physical Therapy license
BCLS (Basic Life Support)
Experience with OASIS documentation
