Shift Timings: 19:00 - 07:30 Mon day to friday (3 Days/12 Hours)
Per Week: 36 Hours
Acute Care experience.
Responsibilities
Implement and monitor patient care plans. Monitor, record and communicate patient condition as appropriate.
Serve as a primary coordinator of all disciplines for well-coordinated patient care.
Note and carry out physician and nursing orders.
Assess and coordinate patient's discharge planning needs with members of the healthcare team.

Requirements:

1. Education & Licensure:

Must be a Registered Nurse (RN).

Relevant state licensure for nursing practice.

2. Experience:

Minimum 2 years of experience in acute care.

Experience in telemetry nursing or similar acute care settings is preferred.

Experience with patient care planning, assessment, and discharge coordination.

3. Skills & Competencies:

Ability to implement and monitor patient care plans.

Ability to monitor, record, and communicate patient condition effectively.

Competency in carrying out physician and nursing orders accurately.

Coordination skills to work with multiple healthcare disciplines for well-coordinated patient care.

Strong discharge planning skills, collaborating with the healthcare team.

4. Work Schedule & Physical Requirements:

Ability to work night shift: 19:00 – 07:30, Monday to Friday.

Able to handle 12-hour shifts, totaling 36 hours per week.

5. Additional / Optional:

May consider traveler RNs (temporary/contract positions).

Strong communication and teamwork skills for coordinating across care teams.
