Soul Care Services is an Illinois-licensed and Medicare-certified home health agency that provides skilled, individualized, and comprehensive home health services such as nursing and therapies (PT, OT, ST), Case Management, Wound Care, and Infusion Therapy. Our multi-lingual staff treat our clients like family and it is our belief that combining expert skills with genuine compassion, we will truly change the delivery of health care and the lives of every person we touch.
