Health Care Partners Foundation, Inc

The Health Care Partners Foundation is a non-profit company specializing in community-driven telemedicine services in rural Colorado and small county detention centers. We provide medical and mental health services as well as addiction support and transition from county jails to the community. The Foundation strives to collaboratively design and implement community-driven systems that integrate individuals into communities that will not allow them to fall through the cracks.
