VNA-Community Services, Inc. The VNA is a 501(c)3 agency with a mission of supporting the integrity and independence of vulnerable individuals and families. Founded in 1919, VNA-Community Services, Inc. operates health and human services programs to improve vulnerable individuals' health, well-being, and independence in Montgomery County, Pennsylvania, and the Greater Philadelphia Community. The agency serves 5,000 individuals each year. Core Programs include a Family Health Center that provides primary care to uninsured and underinsured individuals, an in-home services program that helps frail seniors remain safe in their own homes and communities, and a benefit access program that connects the most vulnerable residents with a connection to up to 23 public benefits and supports. Nurses at the VNA Nurses are the heart of VNA's work. In VNA's Family Health Center, Nurse Practitioners practice to the scope of their licensure by providing primary care services in our clinic and residential areas. While RNs and LPNs support clinical operations and provide community nursing services such as newborn visits, community health screenings, and immunization clinics. In the in-home program, VNA utilizes RN supervisors to provide care oversight for our clients and staff supervision for our home care aides. VNA's work is supported by a social work team that connects patients, clients, and the general public with benefits and supports that will improve their health, well-being, and sense of independence.
