We are dedicated to providing high-quality, accessible healthcare services to our patients through innovative communication solutions. Join our team as a compassionate and dedicated Nurse Practitioner, where we focus on underserved populations confronting self-care challenges and limited medical access to medical care. This role involves conducting patient assessments via Telehealth, ordering medically necessary diagnostics, and conducting patient follow-ups. Your responsibilities include documenting patient progress and maintaining accurate records. To thrive in this role, you should possess a current Nurse Practitioner license in either NJ, GA, PA, MD, NY or TX and be credentialed with the states' Medicaid/Medicare programs, along with excellent communication and collaboration skills.
