Armadale Health partners with primary care practices to provider remote care to their Medicare Patients including Chronic Care Management and general patient support and wellness. We are looking for nurses who enjoy speaking and developing relationships with patients and appreciate remote work with fully-flexible hours. Nurses would perform Annual Wellness Visits on Medicare Patients prior to their office visit and for patients that enroll in the Chronic Care Management program, would assist patients on a monthly basis with various remote support: Nurses would have flexibility regarding how many patients they covered, the best time to contact patients and hence can manage their hours and calendar.
