Role: Remote Advanced Practice Nurse (Neurology)

This role is fully remote but requires a license in the state of Kentucky.
Position Summary
As a Remote Advanced Practice Nurse, you will serve as a primary clinical lead within a physician-led interdisciplinary team. This 100% remote role involves developing and managing integrated care plans that synchronize medical requirements with daily community-based support. You will utilize proprietary technology to provide high-touch coordination for members with complex needs, including technology dependence.
Key Responsibilities
Integrated Care Planning: Lead the development of comprehensive care plans that address both skilled and unskilled needs for members.
Clinical Oversight: Provide remote monitoring and clinical interventions for members with complex needs, such as traumatic brain injuries.
Telehealth Consultation: Conduct virtual assessments and follow-up visits via an integrated technology platform.
Interdisciplinary Collaboration: Work closely with an ancillary network of nursing, therapy, and medical equipment providers to ensure quality care.
Stakeholder Coordination: Act as a clinical liaison for health plans to close quality gaps and reduce total costs of care through proactive management.
24/7 Clinical Support: Participate in a rotation providing clinical guidance and crisis support for members and their families.
Compensation & Benefits
Salary Range: $185,000 – $245,000.

Qualifications
Education: Master’s degree in Nursing from an accredited program.
Licensure: Must hold a current, unrestricted Kentucky Advanced Practice Nurse license.
Experience: A minimum of 5 years of clinical experience, specifically working with complex disability populations or neurology.
Skills: * Proficiency in telehealth technology and remote patient monitoring.
Strong understanding of Home and Community-Based Services (HCBS).
Exceptional communication skills to support families within fragmented care systems.
