A Day in the Life of a Nurse Practitioner at InnovAge
Schedule: Monday through Friday, 8:00 AM to 5:00 PM — no nights, weekends, or holidays!
Your day begins with a warm cup of coffee and a quick review of your participant list. You’ll start by checking in on any updates from the care team — perhaps a participant had a medication change overnight, or there’s a new lab result to review. From there, you’ll spend your morning seeing participants either in our center, or in their homes occasionally, performing physical exams, reviewing test results, and addressing any new health concerns.
Between visits, you’ll chart your findings directly into each participant’s medical record, ensuring every detail is accurate and up to date. You might order or interpret diagnostic tests, draw blood, perform EKGs, or change a dressing — hands-on care is a big part of your day.
As the day goes on, you’ll collaborate closely with our interdisciplinary team — nurses, social workers, therapists, and other providers — to coordinate care plans that address each participant’s unique needs. You’ll manage both acute and chronic conditions, write prescriptions, and provide ongoing follow-up to keep participants feeling their best.
Education is a key part of your role too: you’ll take time to explain treatments to participants and their caregivers, helping them understand their health and how to maintain it at home. Occasionally, you’ll offer emotional or spiritual support to participants and their families, especially during times of stress or transition.
If a medical emergency arises, you’ll step in quickly to assess, stabilize, and guide next steps. Throughout it all, you’ll ensure every interaction and treatment is properly documented in compliance with PACE policies.
By the end of the day, you’ll wrap up your notes, touch base with team members for any follow-ups, and head home — confident that your work made a meaningful difference in the lives of the participants you serve.
REQUIRED
Current State issued Nurse Practitioner license
1+ year as a practicing Nurse Practitioner
DEA Certification with full prescriptive authority
5+ years a Registered Nurse with experience in elderly care
PREFERRED
Bi-lingual
