Advancare Healthcare Services is expanding its clinical team and seeking a dedicated, skilled, and compassionate Registered Nurse to provide exceptional home-based care
to patients throughout the South Side of Chicago. This role is ideal for an RN who values autonomy, meaningful patient relationships, and the ability to make a direct impact on
health outcomes in the community.

What You Will Do

• Comprehensive OASIS assessments that drive accurate care planning and compliance
• Skilled nursing visits including wound care, medication management, chronic disease monitoring, and patient education
• Plan of care development in collaboration with physicians and interdisciplinary team members
• Timely, precise documentation using Axxess Home Health
• Communication and coordination with caregivers, therapists, and providers to ensure seamless care
• Clinical decision-making in the field, using your judgment to respond to patient needs in real time

Who We’re Looking For

• Registered Nurse (RN) licensed in Illinois
• Home health experience preferred, but strong clinical skills and willingness to learn are equally valued
• Confident working independently in the field
• Strong communication, assessment, and documentation abilities
• Reliable transportation and comfort traveling throughout the South Side of Chicago
• A clinician who leads with empathy, professionalism, and integrity
