The Licensed Practical Nurse (LPN) performs a variety of professional nursing duties in compliance with the Comprehensive Home Care regulations and follows established policies and procedures for our beautiful Assisted Living/Memory Care building (Meadow Woods) in Bloomington, MN.

-72 hours in two weeks
-Evening shifts; start time typically 2/230 pm
-Every third weekend and holiday rotation

As part of the Martin Luther Campus Senior Community, Meadow Woods Assisted Living is a residential setting with woodlands, wetlands, the Mall of America, and the Minnesota River Bluffs. Our 113-unit assisted, independent, and memory care buildings are located in East Bloomington, MN near public transportation. Responsibilities/Job Description
-Provides skilled nursing care such as; implementing practitioners’ orders, ordering and assisting with medications and supplies, operating medical equipment, and charting practices
-Lifts and transfer residents as needed, Initiates emergency treatment as needed
-Monitors work and performance of unlicensed staff
-Updates RN with current observation and completes documentation of resident health status
-Communicates changes in residents' conditions to relevant individuals
-May support RN by completing resident assessments, service agreements and care plans
