Description

Clinical Navigator

Location: Kootenai Clinic-Cancer Services

Schedule: Monday-Friday

Position Summary:

~Assists patients in understanding their diagnosis, treatment options and the resources available.

Responsibilities:

~Serves as an essential link between patients and all other medical staff.
~Facilitates appointments for consults and support services within established service standards, including social workers, dietitians and physicians.
~Coordinates multidisciplinary planning conferences, develops concise patient summaries for use by the care team and documents recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines.
~Develops patient education programs and tools.
~Follows patients throughout the care continuum, including inpatient admissions and collaborates with inpatient care management resources.
~Adheres to established productivity benchmarks; identifies opportunities to reduce the cost of care.
~Responsible for outreach efforts to establish and maintain positive working relationships with medical staff.
~Performs other related duties as assigned.
~Familiar with standard concepts, practices, and procedures within the field
~Relies on experience and judgment to plan and accomplish goals.
~Regular and predictable attendance is an essential job function.
~Competent to meet age specific needs of the unit assigned.

Requirements
~BSN required; MSN preferred
~Graduate of a nationally accredited nursing program
~Licensed as a RN by the Idaho Board of Nursing OR licensure pending as documented by temporary licensure OR current RN licensure from a state participating in the "multi-state privilege to practice" compact with Idaho
~Minimum of 3 years of relevant nursing experience required
~Certification within specialty area required within one year of meeting qualifications
~Must complete KH BLS during orientation period
