💼 Job Summary
We are seeking a reliable and dedicated LPN to provide exceptional nursing care in a skilled nursing and long term care environment. The ideal candidate is detail oriented, patient focused, and committed to maintaining high standards of clinical practice and resident safety.

🩺 Key Duties & Responsibilities
• Administer medications and treatments according to physician orders and facility protocols
• Monitor, assess, and document resident conditions; report changes promptly to the RN or provider
• Perform wound care, injections, and other nursing procedures within LPN scope
• Support safe resident transfers and mobility using proper equipment and techniques
• Collaborate with CNAs, RNs, and interdisciplinary teams to ensure coordinated care
• Maintain accurate charting in EHR systems and uphold HIPAA standards
• Provide compassionate communication and support to residents and families

📘 Scope of Responsibility
• Deliver direct nursing care to residents with varying medical needs
• Ensure compliance with facility policies, infection control practices, and safety protocols
• Participate in care planning, shift reports, and interdisciplinary meetings
• Promote a positive, team centered work culture

🕒 Schedule
• Full time, Part time, and PRN opportunities
• Day, Evening, Night, and Weekend shifts available
• Flexible scheduling based on facility needs

LPN Requirements – Pennsylvania
• Active LPN License in PA
• Recent Physical Exam (within 12 months)
• TB Test (2 Step PPD or QuantiFERON)
• Valid Identification Card (Driver’s License or State ID)
• Social Security Number
• COVID 19 Vaccination Card (or exemption if accepted by facility)
• Current CPR/BLS Certification (if applicable to facility requirements)
