Description:
Seeking an experienced Inpatient RN Case Manager (IP CM) for a travel/contract assignment in Glendale, CA. The RN Case Manager will be responsible for managing patient care coordination throughout the inpatient stay, with a strong focus on complex discharge planning, utilization coordination, and transitioning patients safely to the appropriate level of care.

This position is ideal for an experienced CM RN who can work independently with minimal training and is comfortable handling complex cases in a fast-paced hospital environment. Experience working strike-force assignments, particularly with Kaiser Permanente (KP) or HCA California facilities, is preferred.

Schedule:

Day Shift: 8:00 AM–4:30 PM
Sunday–Thursday
40 hours/week
No on-call

Location: Glendale, CA
Weekly Blended Pay: $3,036

Requirements
Active California RN license
Inpatient Case Management (IP CM) experience required
Strong experience with complex discharge planning
Ability to independently manage a complex inpatient caseload with minimal training
Experience coordinating transitions of care, including appropriate post-acute services and placement
Strong knowledge of hospital-based care coordination and interdisciplinary collaboration
Ability to work effectively with physicians, nurses, social workers, patients/families, and post-acute providers
Strike-force experience with Kaiser Permanente or HCA California is preferred
Strong clinical judgment, organization, communication, and documentation skills
Must be comfortable working in a fast-paced inpatient hospital setting
