We are looking for a graduate of an accredited nursing program with an active Oregon RN License. While a BSN or MSN and prior experience in case management are preferred, your commitment to patient care is what truly matters. Qualifications:

Education:

Required: Graduate of an accredited school of medicine, social work, nursing or related medical occupation.

Preferred: Previous experience with coordinating community resources for post-acute hospitalization care. BSN/MSN.

Required: Active Oregon RN License

Preferred: Case Management Certification
EMR: Epic
