Description

LPN- Shift Nurse

1) Assumes Administrative and Skilled Nursing duties as needed on each shift.
2) Works as a team with program staff toward the best interests of persons served.
3) Ensures that individuals being served obtain and maintain their optimal level of health.
4) Implements physician’s orders, MAR (medications, treatments and skills) and individual support plans as directed.
5) Implements the nursing activities of each home assigned to work in.
6) Maintain the home’s safety and maintenance by reporting, implementing, and monitoring for communicable disease or infections or other medical concerns; also completes vitals, body checks, administers medications as ordered or needed, etc.
7) Provides direct care staff with assistance as needed and helps to maintain the cleanliness of the home.
8) Documents all care daily in the required software systems. (Practical Health Systems- PHS)
9) Communicates accurate information to others in a professional and courteous manner, including outside entities.
10) Behavior exhibits professional attitude, demeanor, and interpersonal skills required by position and the company.
11) Maintains compliance with various federal and state regulations and various agency policies/procedures.
12) Other duties as assigned.

Requirements
• Current, valid licensure as a Licensed Practical Nurse with the state of Tennessee.
• Knowledge of direct care nursing and documentation responsibilities.
• Knowledge of patient care charts and patient histories.
• The ability to evaluate therapeutic programs and services for individuals receiving nursing services.
• Strong organizational and coordination skills.
• Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community including with non-licensed medical personnel; the ability to communicate technical information to non-technical personnel.
• Time management and follow-up skills with ability to plan, organize, and prioritize a workload; and handle several tasks simultaneously.
• May be required to attend medical appointments with the individuals for accuracy of medical information disseminated and for safety.
• The ability to make administrative/procedural decisions and judgments.
• Working knowledge of computers.
• Must be at least 21-years of age, must be able to pass thorough criminal background checks, and must pass a pre-employment drug screening.
• Operation of a motor vehicle is required which includes: proof of a current, clean, valid Tennessee driver’s license.
