Our client is looking for a Home Health Physical Therapist in Houston TX!

What you need to know:
The Physical Therapist provides physical therapy services; communicates the patient's progress with other disciplines and directs, supervises and instructs non-professional home health aide staff in the provision of personal care to the patient.

• Under the physician's order, admits patients eligible for home care services within 24-48 hours
• Assess and evaluates patient needs/problems, identifies mutually agreed upon goals with patients
• Reports patient status and need for other disciplines to agency Clinical Supervisor and
referring physician
• Reports to assigned follow-up Clinician as indicated
• Develops patient care plan that specifically addresses identified patient problems and goals

Additional job details:
• Travel with PTA to see 5-7 patients a day (depending on type of care they need)
• Up to 60 mile travel radius
•Will be able to have a lot of 1:1 time with the patients, so ability to create great rapport with patients is key.
