Licensed Practical Nurse - Nurse coordinator

Career opportunity for a reliable LPN (Licensed Practical Nurse) to join our patient care team. You will work with an Internal Medicine Physician and APRN in a private practice office setting. Schedule is Monday-Friday, NO WEEKENDS/HOLIDAYS. The ideal LPN will be experienced in providing medical care and skilled in Population Health Management. It is a fast paced environment, with a variety of responsibilities, where you can truly make a positive difference in the lives of many people. Compensation is flexible and based on experience. Benefits include Health insurance, PTO, 401k, Free parking and some meals.

Requirements
LPN Responsibilities:
-Record patient’s medical history accurately
-Take and record measurements of blood pressure, temperature, heart rate etc.
-Observe patients under treatment to identify progress, side-effects of medications etc.
-Perform Medicare annual wellness visits and monitor patient chronic conditions
-Administer injections, prescribed filled medications and assist with other basic medical care tasks
-Medication refills
-Collect samples for lab testing as assigned
-Communicate with patients’ family or friends to provide advice, comfort and education on chronic medical conditions
-Chronic Care Management and follow up under the supervision of Physician
