StartDate: ASAP Available Shifts: 8 D Pay Rate: $2193.00 - $2305.00

This Home Health RN Case Manager role is based in Vienna, West Virginia, a welcoming riverfront community in the Mid-Ohio Valley. Vienna offers the best of both worlds: a peaceful, small-town atmosphere with friendly neighbors and tree-lined streets, combined with convenient access to shopping, dining, and services. The city sits along the Ohio River, providing scenic views and plentiful opportunities for walking, biking, and outdoor activities. Nearby parks, trails, and recreational areas make it easy to enjoy nature after work, while local shops and restaurants provide a comfortable, hometown feel. As a Home Health RN Case Manager, you will play a central role in coordinating and delivering high-quality nursing care to patients in their homes. You will complete comprehensive assessments, develop individualized care plans, manage ongoing follow-up visits, and collaborate closely with physicians and the interdisciplinary team. Typical responsibilities include skilled nursing visits, medication management, wound care, chronic disease monitoring, patient and family education, and timely documentation in the EMR. Your workday will generally involve traveling to multiple patient homes in and around Vienna, managing a caseload that allows for meaningful, relationship-based care. Patient ratios and visit volumes are structured to support thorough assessments and strong patient engagement. You will have the autonomy to plan your daily schedule, prioritize visits based on clinical need, and adjust care plans as conditions change, while knowing you have access to a supportive clinical team and leadership. Shifts are typically daytime hours, with some flexibility to accommodate patient needs and agency scheduling. On-call responsibilities may be rotated among team members, with clear guidelines and support. You will work in a professional environment that values collaboration, compassionate care, and consistent communication. The role offers significant opportunities to build your skills in case management, community-based nursing, and chronic disease management, making it an excellent career step for RNs looking to expand their experience beyond the traditional hospital setting. In Vienna and the surrounding area, you will find a strong sense of community and a relaxed pace of life that supports both professional focus and personal well-being. With easy access to neighboring towns in the Mid-Ohio Valley, you can enjoy local festivals, outdoor events, riverfront activities, and family-friendly amenities. This setting allows you to make a real difference in the lives of patients while enjoying a comfortable, vibrant place to live and work.
