Position Summary
Under the direction of the Home Care Administrator/General Manager responsible for providing professional medical and clinical home care services for clients. Acts as DON (Director of Nursing) for Home Care Licensing under Class A, Class B and Medicaid HCBS and IHSS Certification. Thorough knowledge of all State and Federal Home Care regulations. Responsibilities include performing direct skilled nursing care, admissions, comprehensive assessments, home visits related to care or supervision, medication management and care plan updates. Ensures care coordination with client physicians, family and/or primary representatives, third party providers and power of attorneys. Evaluates client needs for other services, making recommendations to the Administrator/Client Services Manager when appropriate. Coordinates and confirms medical/skilled scheduling for clients with the Staffing Department. Performs scheduled and unscheduled visits as necessary and communicates plan of care with other team members. Submits accurate and timely documentation. Coordinates with the Client Services Manager and Staff Manager and assists with supervision and direction of care by all medical/skilled LPN and CNA home care staff. Coordinates with the Client Services Manager oversight of Private Duty Nursing assignments to include but not limited to RN, LPN and CNA client assignments. Communicates regularly with team members. On Call RN rotation.

Essential Duties
Performs skilled home care to clients, following physician orders and in coordination with the Administrator/Client Services Manager.
Coordinates client care with the Administrator/Client Services Manager and Home Care RN team.
Coordinates client care with the client physician, family/primary representative and client power of attorney.
Care coordination with other provider entities (i.e. Home Health, Hospitals, Communities, etc.)
Evaluates client nursing and medical needs and develops a plan of care per physician orders.
Responsible for setting up medicinal reminders for clients.
Responsible for coordination with the Administrator/Client Services Manager the training and supervising of the Home Care LPNs and CNAs.
Continually monitors and assess client’s status and reports changes to the Administrator/Client Services Manager.
Participates in In-Services, Meetings and Care Conferences.
Instructs client, family/primary representative, power of attorneys in the client plan of care and refers to community resources as necessary.
Revises and updates Medical, Nursing plan of care as appropriate and every 60 days by Home Care regulations and obtains physician orders and approval.
Responsible for all Medical/Skilled client charts and files.
Responsible for meeting and obtaining Quality Management protocols for all Medical/Skilled client care.
Participates in Rhythms All Staff Meetings and Rhythms Department Continuous Quality Improvement (CQI).
Completes 12 topics of in-service training per calendar year.
Promotes customer satisfaction for all clients.
Takes on-call rotation as scheduled.
Responsible for encouraging, participating and integrating Masterpiece Living and Eden Alternative initiatives by supporting successful aging and person-centered programs and culture.
Other duties as assigned.

Basic Qualifications and Experience
Colorado Registered Nursing License, in good standing.
Minimum of 2 years experience in a home care and home health environment.
Current CPR certification.
Strong knowledge of governmental home care and home health regulations.
Excellent assessment and communication skills required.
Minimum 1 year experience supervising direct reports referred.
Must be able to read, write and speak the English language.
Must possess excellent customer service skills.
Proficiency in Microsoft Office (Word, Excel and Outlook) applications required. Mircosoft Power Point and Publisher preferred by not required. Database experience preferred especially in a business tracking system preferred?.
Must have a passion for serving seniors and a proven track record of excellent customer service.
Must be flexible and possess the ability to manage multiple priorities simultaneously.
Must be able to problem solve, effectively manage challenging situations, and be self directed..
Must be able to read, write and speak the English language.
Must possess excellent customer service, interpersonal, written and verbal communication skills.
Must have a Colorado Driver’s License in good standing.
