Overview of the Role:

As the Director of Clinical Services, you will coordinate and supervise nursing care both in the center and at participants' homes, ensuring exceptional care is delivered 24/7. This pivotal role requires leadership in designing, implementing, and evaluating clinical services, collaborating with a diverse interdisciplinary team to enhance participant satisfaction and health outcomes.


Responsibilities:

Drive participant satisfaction by fostering strong relationships with co-workers, participants, families, and community members.
Spearhead the overall responsibility for clinical services design, implementation, and evaluation alongside the Executive Director.
Lead the hiring, mentorship, and evaluation of clinical staff, ensuring competence and job satisfaction.
Oversee the supervision of Home Care Coordinators, Nursing staff, PT, OT, Social Work, and Registered Dieticians, ensuring high-quality care.
Ensure timely completion of home care assessments and foster collaboration with the Medical Director for effective care delivery.
Establish and lead an efficient 24/7 on-call process to provide essential nursing and medical support.
Participate in interdisciplinary team meetings, enhancing understanding and coordination of participant care needs.
Develop and monitor the clinic’s annual budget, optimizing resource utilization.

Requirements

Requirements:

Bachelor’s degree in a healthcare profession is required; Master's degree in a health-related field and professional certification are preferred.
A minimum of 3 years of experience working with the frail and elderly in acute, long-term, or community-based settings.
Minimum of 2 years of management experience in a healthcare environment.
Familiarity with paperless documentation systems is preferred.
Reliable transportation with a valid driver's license and state-required auto insurance.
Must be medically cleared for communicable diseases and maintain up-to-date immunizations.
Successful completion of a comprehensive background check.
