Job description
Family Nurse Practitioner with 2 years of experience who would like to work a couple of days per week. Active license, DEA and prescriptive authority DEA X waiver able to obtain. Willing to work Monday, Tuesday, or Wednesday hours 8-5 with lunch 12-1 or 9-5 with a 30 minute lunch break. Must have knowledge working with Medicaid , Medicare, and other insurances. Must be knowledgeable with electronic medical records.

Position Summary:To provide effective, efficient and appropriate services to CPC patients in to serve the community’s healthcare needs.

Position Responsibilities:

Assists CPC in developing and keeping up with current clinical guidelines
Participates in provider and medical care team meetings
Establishes positive relationships with local medical community and attends local medical society
Interviews patients to obtain history performs physical examinations, orders labs and other tests, prescribes medications and treatments
Provides continuity in managed care for patients with preexisting long term problems
Makes referrals for secondary and tertiary care
Performs medical procedures based on knowledge of procedures (i.e. toenail removal, I&D, etc.)
Provides health maintenance visits evaluates for immunizations and provides anticipatory guidance and referrals; performs contract and special physical examinations as EPSDT Head start employment, etc.: performs STD and family planning screening and education.
Documents and codes all patient contact accurately and legibly in the EHR medical record
Performs as an active member of the medical team
Performs other duties as assigned i.e. drug reps
Able to manage chronic pain patients whom are established patients
Position Qualifications:

Knowledge of adult learning principles and community resources
Skill in providing excellent customer service and support; organizing and prioritizing workload and excellent written and verbal communication
Ability to interact effectively and professionally with persons from diverse cultural, socioeconomic, education, racial, ethnic and professional backgrounds
Ability to work effectively with managers, co- workers members of the public and professional groups
Ability to communicate effectively, clearly, concisely with others (internal and external customers, both verbally and in writing), consistently demonstrate positive/proactive customer service attitude. Consistently maintains ethical behaviors exemplary of quality public service and fair standards, inclusively, among all employees and members of the public
Ability to work as an active team member and also independently, exercise sound judgement and initiative; be flexible to shift priorities; work toward goals and objectives of draft priorities
Ability to follow CPC clinic practice guidelines-including adhering to the CPC immunization policy
Ability to receive constructive feedback including evaluation of provider’s productivity and practice attributes
Other duties as assigned, including but not limited to participation in providing education quality review and or other activities
Minimum Qualifications:

Must be computer literate with ability to enter information in the CPC EHR system and compile reports or data as requested
