An exciting career awaits you at RNS Private Duty Care! We are currently looking for a Registered Nurse with three or more years’ experience to work full time. The Registered Nurse (RN) Case Manager is involved in initial and ongoing assessments, provides oversight of the care plan, and directs the caregiving team as directed by the client and/or client’s family in accordance with RNS policies and federal/state laws/regulations. The RN Case Manager acts as the Care Manager and is the primary point of contact for the caregiving team, the client, and the clients family. Job Requirements

Qualifications:
Is a graduate of an accredited school of professional nursing and is currently licensed as an RN in the state of Missouri and Kansas with working knowledge of professional standards and principles.
Has a minimum of three years nursing experience, preferably with hospital and geriatric experience.
Must be a licensed driver with a clean driving record and adhere to RNS’ Vehicle Safety Program.
Has a vehicle that is insured in accordance with state and/or RNS’ policy and is in good working order.
Must own a cellphone with Internet and texting capabilities.
Must have daily access to RNS’ email.
Must complete all physical and mental demands, which may include, but not be limited to the following: the ability to transfer clients, carry supplies, stand, walk, use hands, reach, stoop, kneel, crouch, and speak effectively.

Responsibilities:
Develop care plan for client, task list and determine appropriate reporting/charting
Support team members with critical decision making and coaching; responding promptly to calls, texts and emails
Work with the team members and the team leader to coordinate, communicate and execute the care plan
Acts as primary liaison between the team members, team leader and the client and client’s POA or family involved
Coordinates care with physicians, therapies, pharmacy
Maintains shared document and keeps up to date with concerns from team
Oversight of all aspects of care (equipment, refills, documentation, safety)
Coordinate necessary charting and supplies are retrieved from RNS
Maintains regular assessment of client
Provide training to team members on skills as needed, manual BP, glucometer, safe transfer, wound care
Work with the scheduler to explain type of team members needed for client
Address concerns directly with team members as they arise; contact upper management for recurring concerns with employees
Reviews all incident reports
Send team communications to new team members prior to orientation
Submit necessary paperwork if private insurance
Reviews team members charting and notes for accuracy
Perform team members’ annual evaluations
Makes immediate plans for retrieval of RNS equipment and paperwork at the client’s residence when the case ends
Maintains a valid RN Liability Insurance Policy to be updated annually
Has working knowledge of home safety, including physical and psychological dangers, for self, the client, and other team members.
Complies with RNS’ policies and procedures including standards of conduct and work ethics, HIPAA policy, dress code and weekend and holiday availability policies.
Agrees to variable work schedules, including weekend, holiday hours, and client changes.
Complies with all state and local regulations and accreditation standards, including CPR certification.
Establishes and maintains harmonious relationships with the client, the client’s family, and other team members.
Has excellent writing and charting skills and prepares reports in a timely manner.
Maintains objectivity in coping with the stress of working with acute, chronic and terminally ill clients.
Possesses ingenuity to handle medical and situational emergencies.
Is self-directed with the ability to work with little supervision.
