Nurse Practitioner

We are seeking a full-time Nurse Practitioner with a strong focus on quality care, clinical excellence, and delivering patient-centered experiences for our new clinic. Under the guidance of and in collaboration with our Clinic Physician, this Nurse Practitioner will help improve community health by assessing, prescribing, treating, and educating patients as part of patient care delivered in a walk-in setting and/or telehealth sessions conducted from the clinic.

As an evidence-based practice, we leverage proprietary, HIPAA-compliant technology to ensure positive health outcomes and experiences for our patients, therefore this Nurse Practitioner must be also comfortable in providing telehealth sessions (conducted via two-way, video-audio appointments) as part of patient care plans.

Our Nurse Practitioner will be responsible for, but not limited to:
-Performing initial patient screenings, patient history, physical examinations, and preventive health measures.
-Demonstrating proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation of chronic patients required
-Providing in-patient and virtual, video visits.
-Ordering and interpreting health assessments and recommending appropriate actions.
-Prescribing and reviewing medication orders.
-Coordinating follow-up care and collaborating with the Clinic Physician, referring physicians, health providers, and hospital staff, when appropriate.
-Participating in quality improvement activities and developing strategies that help improve patient care delivery, and cost-effectiveness
-Safeguarding patient and Company data.
