Position Title: Travel Case Manager

Start Date: ASAP


Job Responsibilities:

Provide a link between members and available resources to provide the necessary resources in a cost-effective manner.
Going around to members within the community that have been readmitted into the hospital multiple times to perform safety checks, evaluations, and assessments to ensure that they are receiving the right level of care.
Hours: 40 hour work week, M-F 8am-5pm

Education/Experience:

RN certification is required
Requires BA/BS and 3 years of clinical care experience
certification as a Case Manager is preferred
