Under immediate supervision, supports providers (physicians, nurse practitioners and physician assistants) in provision of patient care; includes recording vital signs, discussing chief health concerns, performing testing, and administering injections; assists with procedures and documentation utilizing an EMR.
This employee will work as part of the PCMH team with the following essential duties and responsibilities:
Patient care: records vital signs, discusses and records chief health concerns, allergies and obtains and documents patient historical health information in EMR; performs in-house lab tests; administers injections, stocks patient rooms with supplies.
Service Collaboration: contacts pharmacies to order/re-order prescriptions; sets-up referral to specialty care and other community resources; collaborates with other service providers and colleagues to provide comprehensive services to patients.
Administrative duties: answers phone; acts as patient advocate; attends regularly scheduled meetings, performs other general office duties as required.
Quality Assurance: participates in the CQI process.
Education: conducts patient education groups/sessions; collaborates with the health education team to develop health curricula and patient education materials; conducts health fairs.
Other duties assigned within LPN scope of practice and appropriate in service- training
Minimum 5 years experience of community health center work experience.
Current CPR certificate is required.
Working knowledge of EMR.
Knowledge of medical terminology.
Knowledge and understanding of HIPAA regulations and applicable confidentiality laws.
Ability to maintain schedules.
Ability to file and retrieve information.
Excellent PC skills in MS Office: Word, Excel, PowerPoint & Outlook.
Excellent communication and interpersonal skills.
Ability to work independently with attention to detail.
Ability to respond to questions in a tactful and professional manner.
Ability/willingness to adhere to the established departmental service standards.
Ability to exert physical effort, standing, lifting and carrying materials or equipment.

Care for the Homeless advocates for policies to ameliorate, prevent and end homelessness. We occasionally circulate policy information to employees and require that staff review it. While we don’t require it, we encourage every employee to become involved in supporting and advocating for our agency’s policy agenda and related activities.
Associate degree from an accredited Nursing Program or College.
Valid LPN license in NYS is required.
