The Director of Clinical Performance will help support the analytic, strategic, and collaborative work across OneCity Health partners and stakeholders needed to build a strong clinical improvement processes as a fundamental part of the integrated delivery system. This role includes but is not limited to the support of quality improvement activities for integrated clinical services across acute care hospital, ambulatory settings and community partners.
The successful candidate will have experience within the clinical delivery system redesign and the ability to work in support of primary care teams in accomplishing the goals of OneCity Health clinical transformation initiatives while strengthening the primary care team’s capacity to successfully implement performance improvement activities utilizing evidence-based approaches.
As a Core Responsibility in the field, the candidate will be able to engage multiple stakeholders within clinical sites, including primary care team members who care for patients, as well as administrative and executive leadership for OneCity Health projects engaged under Chronic Disease Initiatives and Clinical Redesign. Within OneCity Health, the candidate will be able to work effectively in a highly matrixed environment with colleagues who represent expertise in a range of areas that interface with clinical performance improvement.
Core Job Responsibilities:
Under the direct supervision of the Assistant Vice President for Chronic Disease Initiatives & Clinical Performance, the Director of Clinical Performance will be responsible for planning, implementation and to provide ongoing performance management for initiatives that support the OneCity Health partner network in preventing and managing disease
Collaborating with colleagues at OneCity Health partner organizations to support planning for clinical quality improvement and clinical integration across care settings, including researching best practices based on current evidence based standards and guidelines and creation of impactful learning experiences for PPS partners
Leading site visits across the OneCity Health catchment area to support primary care teams in performance improvement activities including: o PPS Partner workgroup attendance o Integration of evidenced-based quality improvement methodology, o Practice goal setting to achieve transformation & redesign using patient centered medical homes standards
Lead, develop and maintain effective working relationships with engaged DSRIP project site contacts and internal support through regularly scheduled site visits and scheduled meetings/calls.
In collaboration with the Assistant Vice President for Chronic Disease Initiatives & Clinical Performance, the Director of Clinical Performance will be responsible for managing special projects for OneCity Health Services to provide strategic insights in development of a strong sustainable Integrated Delivery System (IDS) across all OneCity Health hubs, including but not limited to clinical services, care management, pharmacy, and laboratory service\
Qualification in a clinical field (Professional Registered Nurse Required)
Masters in Science/Nursing (MSN) required
Minimum of 7 years’ experience working in a clinical acute care environment and ambulatory care setting
Current New York State Licensure (ex. Registered Nurse, Nurse Practitioner, Adult Nurse Practitioner)
Excellent interpersonal skills
Experience with concept and implementation of performance improvement approaches and methodologies
Ability to engage effectively with a wide range of stakeholders across multiple health care settings and facilities
Ability to translate complex strategic and operational considerations into concrete plans for health services development
Experience with New York City’s healthcare landscape.
Advanced skills in Microsoft Office applications including Excel, PowerPoint, and Word.
Creative problem solving skills.
Excellent written and oral presentation skills, with the ability to engage, inspire, build credibility and engender trust across all levels of an organization.
Ability to prioritize responsibilities and handle multiple projects simultaneously
