Position Summary:
The Intake Specialist reports to the Intake Coordinator and processes and transcribes home care referrals and enters data into clinical information system. The Intake Coordinator is readily available, through the Agency’s office, during hours of operation, to advise referral sources and members of the staff. The Intake Coordinator coordinates with referral sources and Agency staff to assure adequate information is obtained. The Intake Specialist position is a non-exempt position.
Required Experience for Intake Specialist:
High school education (or equivalent); College degree preferred.
Demonstrates communication and organizational skills, accuracy, flexibility, and ability to work with minimal supervision.
Compliant with accepted professional standards and practices.
Ability to read and interpret clinical documents, such as discharge plans, physician progress notes, and orders.
Avid team player.
Consistently maintains a positive attitude which promotes team and optimal performance.
Accountability for Intake Specialist:
Accountable for accurate transcription into clinical information system.
Adheres to privacy and security standards provided by all federal, state, and local authorities of the Health Insurance Portability and Accountability Act (HIPAA) and other applicable laws.
Promotes a positive and effective system of communication with the Intake Coordinator and others throughout the Agency.
Responsibility for Intake Specialist:
Process referrals and initial communication with patient and/or patient representative for home care admission or resumption.
Serve as a resource to case managers and physicians concerning home health, availability, and admission procedures.
Adhere to the policies and procedures related to referral, intake, and data entry.
Assist in all administrative areas of the department, as needed, including insurance verification, and obtaining authorization, as needed.
Maintain departmental computerized tracking for specific database reporting.
Provide education to referral sources and Agency staff related to necessary items in processing referrals.
Reports complaints, threats to timely delivery of care, and incidents or accidents to the Supervisor, Administrator or Designee immediately.
Compliant with all applicable laws, regulatory requirements, standards of practice, accreditation standards, and policies and procedures.
Assure all adequate information for referral is complete.
Reports cases of possible abuse, neglect, fraud, noncompliance, and exploitation to the Compliance Officer, Administrator or Designee immediately.
Other duties as assigned by the Intake Coordinator.
