Summary
Provide skilled nursing care and support to patients in their homes, developing and implementing personalized care plans to promote health, manage chronic conditions, and improve quality of life.
Responsibilities
Conduct comprehensive health assessments to evaluate patient needs and develop individualized care plans.
Administer medications and treatments as prescribed by a physician.
Perform skilled nursing procedures, including wound care, dressing changes, and IV therapy.
Monitor patient conditions, vital signs, and progress, documenting all findings accurately in the patient's electronic health record (EHR).
Educate patients and their families on their health condition, medication management, and home care strategies.
Coordinate with physicians, therapists, and other healthcare professionals to ensure seamless, comprehensive care.
Provide emotional support and communicate patient updates to the healthcare team.
Assist with personal hygiene and ensure patient safety in the home environment.
3 years of critical care, acute care, or medical surgical RN experience
BSN required
VA or compact license required
