The Family Center’s service array includes integrated Outpatient Mental Health and Substance Abuse Treatment, Legal Services, as well as a wide range of supportive services to assist patients with meeting their treatment and life goals. The Certified Community Behavioral Health Clinic Nurse (CCBHC Nurse) sees patients, conducting health screening and monitoring and providing health physicals. The CCBHC Nurse promotes an interdisciplinary approach to care and ensures integration of services within the agency and with collaborating providers. The CCBHC Nurse provides consumers with treatment education and support and is tasked with motivating consumers to remain connected to all care, including primary medical care. The CCBHC Nurse reports to the Deputy ED for Behavioral Health & Supportive Services.

• Provide health monitoring and conduct health physicals for Certified Community Behavioral Health Clinic (CCBHC) consumers.
• Work collaboratively with treatment team to ensure consumer’s integrated treatment plans address physical health needs.
• Serve as CCBHC liaison with health provider partners (Designated Collaborating Organizations), including FQHCs, the Veteran’s Administration Health System, and area hospitals and treatment facilities.
• Assist with linkage to medical care for individual consumers and participate in discharge planning for consumers being released from facility care.
• Provide consultation and education to CCBHC treatment team to strengthen all members’ understanding of prevalent consumer health needs and common barriers to adherence.
• Assist with planning and implementation of strategies to improve individual consumer compliance with medical treatment.
• Ensure care meets all Trauma Informed Best Practice criteria.
• Promote an interdisciplinary approach to service delivery.
• Work collaboratively with project leadership and supportive resources to ensure that patients have access to comprehensive patient and family health educational programs.
• Collaborate with other members of the project team in development, implementation, and ongoing review of policies, procedures, and standards of care.

Job Requirements

• Education: Bachelor’s Degree in Nursing; Certification in specialty area preferred
• NYS current nursing licensure
• Minimum 4 years of nursing experience, preferably including experience in an Integrated Care setting.
• Minimum 2 years of experience working with impoverished communities whose members are at high risk for chronic health conditions.
• Spanish bilingual, preferred.
