The hospice Nurse Practitioner works in collaboration with the hospice Interdisciplinary Team (IDT) and is directly accountable for making clinical judgments using independent and interdependent decision making skills. He/She is pivotal in identifying the physical, psychological, social and spiritual needs of the patient and family as well as working with the IDT to determine hospice appropriateness. We have openings all across the state of New Jersey.
•Provides consultation in palliative care, symptom management and supportive care to meet the needs of the patient as requested by the Team Leader.
•Conduct face to face visits with our hospice patients to determine continued eligibility for hospice services.
•Consults and coordinates with the attending physician, the hospice medical director, and other members of interdisciplinary team to contribute to the patient’s plan of care.
•Prepares and maintains accurate patient records, charts and documents.
•Assists in the development of clinical practice guidelines / standards in support of quality palliative care as needed.
•Develops and updates plan of care with the IDT and others involved in the patients care.
•Provides skilled nursing as determined by the plan of care.
•Educates patient/family and other primary caregivers, in appropriate care techniques.
•Serves as the patient/family advocate by maintaining regular communication with attending physicians, long term care facility staff, HMO case managers, and others.
•Ensure and document appropriate use of medications, durable medical equipment and supplies.
•Responds to on-call issues as clinically appropriate.
•Promotes a customer service oriented approach to care & customer relationships.
•Current Nurse Practitioner license and registration in New Jersey
•Masters Degree in Nursing
•A minimum of one year nursing experience
•Excellent oral and written communication skills
•Access to reliable transportation.
