• Complete the nursing portion of the MCO initial and comprehensive assessment and complete the MCP according to standardized procedures (within first 10 days and then 6 month intervals.)
• Establish and monitor wellness and health promotion activities based on the Member’s assessment and identified personal and clinical outcomes.
• Re-evaluate the Member’s health and wellness, based upon Family Care standards and nursing best practices, to manage ongoing health and safety issues.
• Coordinate with primary and acute care systems for Members.
• Monitor and follow-up with Member hospitalizations.
• Perform routine health monitoring activities, based on the Member’s need and MCO standards, for example, reviewing blood sugar monitoring results.
• In crisis situations provide appropriate assessment of psychiatric, behavioral or medical problems and implement interventions in conjunction with other members of the CMU Interdisciplinary team and Member supports, the Member’s physician and/or other community support professionals.
• Evaluate Members’ ability to safely manage, either by themselves, or through use of supports, their medication, medical care interventions and treatments, medical equipment, and access to health care services and educate as needed.
• Take appropriate measures to prevent the spread of infection and communicable diseases and instruct the Member and/or caregiver in these measures.
• Ensure that the Member’s physician or appropriate medical professional is notified when the Member has an acute change in condition.
• In collaboration with the other IDT staff, provide or arrange for services, supports or medical care that best support the Member’s identified outcomes, and monitor the effectiveness of these services and supports
• Update the Long Term Care Functional Screen for Members per guideline.
• Promote Members’ independence in functioning at home and in the larger community
• Assist in the coordination of Member natural and formal supports.
• Identify individualized member outcomes and the most cost-effective services/ items needed to achieve these outcomes.
• Complete member centered plans (MCPs) based on assessment findings, supporting medical information, member preference, and the Resource Allocation Decision (RAD)..
• Follow quality standards of practice established by the MCO.
• Provide or arrange for services and supports that best support the Member’s identified outcomes and monitor the effectiveness of these supports and services.
• Update the Long Term Care Functional Screen for Members per guideline.
• Participate in Agency meetings and in-services as appropriate.
• Perform other duties as assigned for the smooth and safe operation of WCS Inc’s and their clients.

Job Requirements

• The Registered Nurse must have current licensure to practice in Wisconsin (A bachelor’s degree (BSN) is preferred) and have at least one (1) year of paid, formal (non-family ) experience working with a specific population (e.g. DD, ID, PD or Frail Elder) in community care management or related community experience with persons in the MCO’s target population. This experience must be directly related to the population the nurse will be serving within the CMU.
• Knowledge of community resources and long term care options available to meet the needs of the members served by the MCO.
• Strong computer skills. Proficient in Windows and Microsoft Office applications. Ability to use computer programs to perform essential job duties.
• Eligible to perform functional assessments with the Long Term Care Functional Screen (LTCFS) and to maintain certification as a WI State screener.
• Able to work independently and as a part of an interdisciplinary team.
• Must have a valid WI Drivers License, own auto, and automobile insurance
• Excellent business communication skills, written and verbal required.
• Not be convicted of a misdemeanor or felony directly related to providing personal care or other health care services.
