The Home Health Services Director is a registered professional nurse who is approved by the Governing Body at the recommendation of the Administrator to organize and direct the Agency's clinical service programs.
JOB RESPONSIBILITIES AND COMPETENCIES
Oversees the clinical management of AZRA’s clinical department
Assists the Administrator in planning the overall development and administration of AZRA as outlined in the policies and procedures
Participates in the development of administrative policies and procedures relating to AZRA home health
Directs implementation of approved work methods and procedures that reflect elements essential to rendering high-quality care
Participates in the review/analysis/appraisal of the effectiveness of the total AZRA program
Utilizes the Outcome-Based Quality Improvement/Outcome-Based Quality Monitoring (OBQI/OBQM) data reports to quantify AZRA’s home care performance against the national norms. Recommends to the Administrator methodology to change outcomes and improve OBQI/OBQM scores.
Oversees the implementation and evaluation of patient care services
Aggregates record review summaries for Professional Advisory Committee (PAC) meetings
Works closely with the recruiter in achieving satisfactory staffing leadership in promoting and maintaining standards for giving high-quality care by all
Assures that established Quality Assurance Performance Improvement (QAPI) committees maintain the goal of meetings
Assists the Administrator in planning/conducting annual PAC requirements
Assists the Administrator in collecting data for Annual AZRA Evaluation/Annual report
Recognizes leadership potential/offers opportunities for leadership training/development
Manages the clinical departmental budget seeking out supplemental funds before exceeding the budgetary requirement
Evaluates own job performance and utilization of resources in planning for professional growth
Receives and acts to resolve complaints subject to the agency’s grievance procedure in the capacity of “next higher level authority.” Apprises Administrator of potential legal risks
Participates in hiring of home health staff
Participates in employee termination process assuring documentation exists to support actions
Ensures implementation of AZRA’s continuing education program
Supervises staff (RN, PT, OT, ST, HHA or any other home health staff positions) as program grows
Projects a concerned, professional attitude and develops a positive rapport with all staff
Establish mutual goal setting and achievement of standards
Develops an open, positive rapport with community resources affiliated with the agency
Maintain high visibility and availability while in the office
Assists the Marketing department as requested
Serves as a member of the PAC
Participates in local organizations and activities related to home health care
Participates in state and national organizations, meetings, seminars, workshops, and activities Accommodates state/accreditation surveyor/staff during site visits
Oversees assignments for patient admissions according to patient needs
Other duties as assigned
A registered nurse with at least three years of nursing experience, including at least 24 months of experience as a registered nurse providing home health services.
Bilingual in English/Spanish
Two years in a management capacity involving significant interaction with people and organizations with diverse and varied interests and backgrounds.
Computer Skills: MHS, MS, Internet, Outlook
Be licensed by the Arizona Board of Nursing as a Registered Nurse (RN) according to A.R.S. Title 32, Chapter 15.
Be at least 21 years of age.
