RN Case Manager, Home Health - Retention Bonuses Offered!

We seek RN Home Health Case Managers to join our dedicated Home Health team. These positions are based in the Eastern and Western suburbs of Cuyahoga County, plus Summit, Medina, Geauga, Lake and Lorain Counties.

Among the key responsibilities:

Direct Care Role:

*Provides direct care services to patients in their homes
*Utilizes principles of the nursing process to assist in data collection and evaluation of nursing care.
*Utilizes effective interviewing techniques; sets limits, and collects data based on specific role.
*Demonstrates nursing knowledge and skills in the implementation of prevention, therapeutic and rehabilitative nursing care.
*Communicates and collaborates effectively with health care providers, third parties, patients and families based on specific role.
*Explains processes, procedures and treatments to patient to gain cooperation, understanding, and ease apprehension.
*Maintains awareness of comfort and safety needs of patient.
*Observes patient, records significant conditions and reactions, and notifies supervisor or Physician of patient's condition and reaction to drugs, treatments, and significant incidents.
*Responds to life saving situations based upon nursing standards, policies, procedures, and protocol.
*Uses oral and written communication to convey pertinent information to members of the health care team in a timely manner.
*Complies with company policies and procedures.
*Completes all required forms, reports, time cards, itineraries, etc, accurately and within designated timeframe.
*Recognizes and follows established lines of communication.
*Performs other duties as assigned.

Indirect Care Role:

*Maintains knowledge of Home Care/Home Base computer system for care delivery applications.
*Coordinates communications with third party payors for pharmacy, DME, and other supplies needed for the clinical plan of care.
*Maintains knowledge of regulatory, licensing and accreditation agencies.
*Performs other duties as assigned.

The ideal candidate for the RN Home Health Case Manager position will have the following qualifications:

*Completion of an accredited RN education program with a diploma, associate or bachelor's degree. Bachelor's degree in Nursing preferred.
*1 year community health experience with diverse populations. Specialty experience with home health nursing preferred.
*1 year experience with providing phone support and/or triage intervention in community health, clinic setting, and/or emergency room preferred.

*Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds. The ability to work in a complex institutional structure requiring significant collaboration.

*Strong planning and organizational skills.

*Demonstrated ability to work well under pressure, within strict time parameters, and with a strong attention to detail.

*Ability to work independently with minimal supervision.

*Demonstrated ability to maintain strict confidentiality.

Required Licenses, Registrations and/or Certifications:

Registered Nurse; licensed in the State of Ohio.
CPR certification is required.
Valid driver's license and working vehicle
