We are seeking a full-time Registered Nurse (RN) to join our team of professionals. This position is responsible for conducting in-home assessments and re-evaluations, assisting in preparing care plans, performing medication set-ups, supervising HHA's and CNA's. The full-time RN position will be responsible for identifying, creating, and following the home health care plan for admission under a home health episode/authorization while ensuring to educate patients, families, caregivers, and other providers to safely perform care. This position will have the opportunity to collaborate with physicians, clients, their families, and CareAparent’s Home Health interdisciplinary team. Coordination and assignment of client visits will be done using centralized scheduling to monitor achievement of 32 visit hours per work week.
